Third Party Services Information and Consent for Parents and Guardians
Bertram Primary School (BPS) uses a number of third party online tools and apps. These services require us to share some personal information about your child. These third party services have been reviewed by the Department of Education and has been classified as per below:
Notification The school is required to notify parents and guardians that a specific service is being used at the school.
Bundled Consent Services in this category require bundled consent – one consent for all services listed.
From the beginning of the school year, students will not have access to personal or school iPads until Third Party Services consent has been provided and been signed by a parent or guardian.
As the Department of Education continues the review of third party services, we will need to gain your consent for additional online tools and apps as they are approved for use within our school.
Thank you to the families who have already completed the online consent forms. Due to the large number of third party services which have been reviewed by the Department of Education since this process began, we ask that you complete and return these forms to supplement the information you have already provided.

Third Party Services Requiring Bundled Consent or Notification Form
I consent to my child’s information being provided, if required, to each of the service providers listed in the linked attachment table while enrolled at Bertram Primary School.
I will be advised by Bertram Primary School if there are any changes to the way a third party online tool collects and/or stores personal data.
I understand I may change my consent for an individual tool or app at any time by contacting Bertram Primary School by email bertram.ps@education.wa.edu.au
